From 1st July 2026, MWL will provide Adult Community Services in West Lancashire.

Our Teams

Our expert teams provide diverse and essential medical, nursing and therapeutic care to patients aged 18+ throughout West Lancashire.

Working to help people plan, manage and adapt to changes in their health from their own home, alternative settings close to home and in community clinics. Our aim is to prevent avoidable admission to hospital and to minimise hospital stay.

 

 

Community Cardiac Team

About the service

The service offers care and support to patients with cardiac conditions, mainly heart failure, particularly after they have been diagnosed, admitted to hospital, or undergone recent medical treatment. This care pathway involves coordination with the patient's GP and/or hospital consultant.

Conditions may include heart attack, valvular heart disease, coronary artery bypass graft, heart failure, cardiomyopathy, angioplasty, stents, ICD, and/or biventricular pacemaker implants.

Community Matrons understand that looking after your health and arranging support can sometimes feel challenging.

Our skilled and experienced team can provide health assessments, advice, education, and ongoing support to help patients manage long-term conditions. This maintains independence and reduces the need for hospital admissions.

Community Treatment Rooms

About the service

The Treatment Rooms are a clinical setting in the community for wound care, removal of sutures (stitches, staples, and clips), injections, ear irrigation, non-urgent Doppler assessment, compression and catheter care.

Patients can self-refer or be referred through their GP Practice and/or hospital.This service is not available for patients who are housebound.

Continence Specialist Nurses

About the service

This specialised team provides individualised treatment programs, education, and support for adults who have bladder or bowel problems from community clinics, care homes and patients’ own homes.

Patients may be offered a comprehensive continence assessment with referral to other health professionals for further investigation and treatment if and when required. Appropriate aids and products may be provided to help manage unresolved incontinence, this will be based on a patients clinical need.

Where symptoms of bladder or bowel dysfunction cannot be improved, the team can support patients to successfully manage this long-term condition.

Diabetes Specialist Nurses

About the service

The Diabetes Specialist Nurses and Diabetes Nurse Educators work alongside dietitians, GPs, practice nurses and diabetes consultants to ensure a comprehensive service for all diabetes patients.

The team is based at Ormskirk Diabetes and Community Services Centre with home visits available for housebound patients.

Appointments

All appointments must be booked in advance.

Alternatively, we can provide telephone advice.

Information about diabetes and further patient education is provided by the Diabetes Specialist Nurses. This is for both newly diagnosed and for more established patients.

District Nurses

About the service

The District Nursing Service provides care, treatment and support to housebound individuals who have an identified physical need for a nursing intervention. Patients must be at least 18 years old and registered with a West Lancashire GP.

How to access the service

The District Nursing service operates an open referral system from patients, carers, health professionals and public sector partners. The patient must be considered to have a specific health or nursing need that requires the expertise of the District Nursing Service.

Referrals for a place of residence visit must only be for a patient who is unable to leave the home, or if it is considered that the place of residence is the most suitable environment for the episode of nursing care to be delivered.

Before referring a patient to the service please gain and record their consent.

Phlebotomy (Blood Collection)

About the service

The community phlebotomy service provides local appointment-based blood tests ordered by your doctor, saving you from travelling to a main hospital.

To ensure a smooth visit:

Bring your form, you must bring the paper blood test request form from your GP or referrer to your appointment.

Book and appointment, your referrer or phlebotomist can book your appointment or you will be asked to call the Care Co-ordination Hub, using the information below.

Please Note - Blood forms must be brought to the appointment. If you do not have your form, the team will be unable to carry out your test.

Respiratory Specialist Nurses

About the service

This multi-disciplinary team offers home oxygen assessments and support services to patients with respiratory conditions across West Lancashire.

The Respiratory Specialist Nurses also deliver the Community Pulmonary Rehabilitation Programme for patients across Skelmersdale.

Urgent Community Response Service (UCR)

About the service

Urgent Community Response (UCR) provides urgtent care to people in their own homes which helps to avoid hospital admissions. Through this service patients who urgently need care can get fast access to a range of health and social care professionals.

This service is provided from 8am to 8pm, 7 days a week.
