Children’s Community Nursing Outreach Team

CCNOT aims to provide high quality, safe care to children and families in the community setting. Our emphasis is on child and family centred care and we empower, encourage and support parents/carers to look after their child collaboratively and together we meet their health needs.

Children recover much quicker at home in their own environment and by looking after children at home it reduces stress, anxiety, cost for families and disruption to the whole family unit.

The CCNOT service is commissioned by North Sefton and West Lancashire Integrated Care Board’s and will take referrals for children/young people who have a GP in these commissioning areas.  Referrals are also taken from the Ormskirk Paediatric Unit – Children’s A&E, Children’s Ward, Paediatric Assessment Bay, Paediatric Outpatients, Ormskirk Neonatal Unit/Post Natal Ward and any tertiary hospital. We will also take direct referrals from North Sefton and West Lancashire GP’s to prevent attendance/admission to hospital.

We provide a 7 day, 365 days per year senior nurse service, and the hours are Monday to Sunday 08.00am to 9.00pm

CCNOT aims to reduce length of hospital stay / promote early discharge, reduce Accident & Emergency / Paediatric Assessment & Observation Unit / Children’s  ward admissions and improve patient and family experience.

CCNOT referrals can include Wound / burn reviews and dressing change, IV/IM antibiotic therapy, review of acute respiratory illnesses, gastro diagnosis for fluid balance, cellulitis review, eczema management, monitoring of faltering growth / babies with weight loss, IM injections., central venous line / portacath flush & dressing change, nasogastric tube support, reflux management, venepuncture.
